DEERS/RAPIDS Identification Cards

PROOF OF IDENTIY IS MANDATORY

Two (2) valid, unexpired forms of identification must be presented to receive any ID card. For a list of authorized proofs of identity (I-9 Lists of Acceptable Documents), please click here.

All documents must be originals or they must be certified or notarized copies.

All Foreign documents must be originals with an Apostille and translated in English

All court documentation must be state or county certified stating that it has been filed and recorded in the courthouse.

When a Social Security (SS) number is required, you may use a SS Card or other official documentation showing the SS number (i.e. Tax ID form, letter from SS Office with SS number or Military ID).

Driver’s Licenses must have an actual date of expiration. 

 

ELIGIBILITY REQUIRMENTS

Military Members

Official documentation must be presented for any updates or changes (i.e. Promotion Orders, Active Duty Orders, Name Change Memorandum, Medicare Card or Official Notification Letter from Social Security Administration, etc)

Must present two (2) valid forms of identification. (I-9 Lists of Acceptable Documents above)

 

DOD Contractors

All data must be previously input through Contractor Verification System (CVS).

If going overseas, a Letter of Authorization (LOA) must be presented reflecting this information , for initial card LOA cannot be older than 30 days

Must have two (2) valid, unexpired forms of identification (I-9 Lists of Acceptable Documents above)

 

DOD Civilian

All data must be previously input through Civilian Personnel Advisory Center (CPAC)

Must have DD 2365 and SF 50

Must have two (2) valid, unexpired forms of identification (I-9 Lists of Acceptable Documents above)

 

Family Members


Must have two (2) valid, unexpired forms of identification (I-9 Lists of Acceptable Documents above) and one of the following:

Sponsor present

Valid Power of Attorney (POA)

DD Form 1172 (ID Card Application) signed by the Sponsor and a Verifying Official.

DD Form 1172 (ID Card Application) signed by the Sponsor and a Notary Public

 

Retirees (Initial Retirement)

Retirement Orders or DD Form 214 (Copy 4)

Reserve/National Guard Retirees need Orders/Letter transferring to Retired List and authorizing retired pay

If Medicare eligible, must present Medicare Card

Must have two (2) valid, unexpired forms of identification (I-9 Lists of Acceptable Documents above)

 

100% Service Connected Disable Veteran (DAV)

Letter from Department of Veteran Affairs showing member as an honorably discharged veteran rated as 100% service connected, authorizing commissary store and exchange privileges

DD Form 214 (Copy 4)

Must have two (2) valid, unexpired forms of identification. (I-9 Lists of Acceptable Documents above)

If adding other family members to the system, see Adding Family Members section below

 

Full-Time Students (21-23 years old) – Attending accredited institution

Sponsor must be providing over 50% support – must be stated in block 89 of the DD 1172

Must have two (2) valid, unexpired forms of identification (I-9 Lists of Acceptable Documents above)

Letter from Student Registrar’s Office or National Clearing House (Enrollment Verification Certificate) that states the following:

1. Full-Time Status

2. Obtaining associate’s degree or higher

3. Anticipated date of graduation

4. The begin date of eligibility shall be the date the sponsor arrives at the ID Card Facility to input the child as a full-time student NOT the date the child enrolled in college.

 

NOTES

If student falls below full-time status, the ID card must be turned in to local ID card facility. 

Only newborns and foreign nationals can be enrolled without a Social Security card. Card must be provided once received to continue receiving medical benefits.

Ward

Court order granting custody to the Sponsor. Court order must stipulate that the child will be in the care and custody of the Sponsor for no less than twelve (12) consecutive months. The court order can designate length of custody by age, time or permanency.

Voluntary Acknowledgment of Paternity from the State in which the child was born

Children receive an ID card at age 10 unless DUAL Military, Single Parent or Illegitimate child.

Incapacitated Child/Parent/Parent-in-law/Ward Approval
The ID Card Facility does not handle the approval for these packets. Packets can be picked up at our customer service window. Once completed you must mail or fax DFAS Indianapolis Center. If you have any question call 317-510-1640/1621/1616/1617/0988/1910.
Upon Approval

Parents must bring in Photo ID, SSN , DD 1172, DD 27-88 and birth certificate of sponsor

Parent-in-law must bring in Photo ID, SSN, DD 1172, and DD 27-88. Marriage certificate of sponsor and spouse along with birth certificate of spouse

Incapacitated child photo ID, SSN, DD1172, DD 27-88, medical sufficiency statement (memorandum from doctor)

Ward must bring in birth certificate, SSN, (Photo ID if over 18 yrs of age), DD 1172 and DD 27-88

Package must be recertified every 4 years

 

LOST OR STOLEN ID CARDS

Two (2) acceptable forms of ID from the I-9 Lists of Acceptable documents along with the documentation below are required to receive a replacement ID Card for ALL Lost or Stolen ID Cards:

Military Members: Memorandum or Counseling Statement from the Soldier’s Commander, 1SG, or Adjutant describing the date, place and circumstances of the loss or theft.

DOD/DA Civilian Employee: a MP/Police Report or a memorandum from the employee’s HR department or supervisor describing the date, place and circumstances of the loss or theft. 

DOD Contractor Personnel: a MP/Police Report or a memorandum from the contractor’s COR, COTR or CVS Trusted Agent (TA) describing the date, place and circumstances of the loss or theft. 

Family Members/Dependents: a MP/Police Report describing the date, place and circumstances of the loss or theft. Note: Sponsor must be present or must have a Power of Attorney.

Retirees: a MP/Police Report describing the date, place and circumstances of the loss or theft.

A police report can be obtained from the Provost Marshal Office (PMO), located at Building 2-5634, on Armistead St. Directions are provided below.
